4/4/2023 EnchantaVaughn Approved 1.Behavior: Upon QMHP-Câs arrival, Client was able to cooperate and communicate well during the session. Client was in a pleasant mood and presented good behaviors, as evidence of her smiling and getting excited for the session. Client was excited to express her feelings and share her favorite thing she loves about having counseling as a way to help her get through challenging situations and work on her attitude. 2. Behavior: During the second portion of the session, client appeared to be anxious with QMHP-C as well as with her mother when discussing how she really doesnât not follow the directives given by her mother and how many times her mother has to prompt her to complete simple task within the home. 1.Intervention: QMHP-C engaged in conversation with Client about her day. QMHP-C administered psycho educational activity with Client and assisted when needed. QMHP-C provided new skills and strategies to help reduce impulsive and hyperactive decisions. QMHP-C taught Client effective communication and assertiveness skills to help her successfully resist negative peer pressure. Examples of assertive, effective communication were provided (e.g. I have to leave nowâ; âI canât afford to get into any more troubleâ). 2. Intervention: QMHP-C encouraged Client to maintain her behavior and told Client that QMHP-C was proud of her. QMHP-C offered simple praises to Client for exhibiting the appropriate behaviors while in the home. QMHP-C assisted Client in monitoring her own behaviors in efforts to promote exercising self-control in focused areas such as respecting boundaries with others. 1.Response: Client stated that she felt she was smart and kind to her family. Client communicated that it is often hard to remain positive at all times about herself when she doesnât always have the best things or necessities that she needs, especially since not being able to go anywhere or do anything. Client discussed numerous situations where she had to adapt even when she didnât want to, such as having to away from friends and loved ones for the last few months. 2. Response: Client acknowledges expectations and reviews goals and objectives with counselor and parent daily. Client was acceptive of prompts given by mom and counselor. Client agreed to listen to the adult and follow all rules no matter if she sees someone else breaking them. Client is reminded to tell an adult if someone is interrupting her learning and mental health in anyway.
